                The assessment is formatted as a table with 12 questions.  The first row of each question is the question statement, 
                the next row labels the columns below with the left hand column being the answer options.  It is labeled “Choice of Action” 
                or “Best Description” or something similar.  To the right the label is 1st choice with an asterisk since you have to choose
                one first choice. To the right of that is the 2nd Choice column which is optional.  Below this label row are the four options,
                one in each row and the column to the right of each option is the radio button for first choice and the last column to the 
                right is the button for 2nd choice. 
                In completing the assessment, you must choose a first choice answer for each question.  If you want to change your first 
                choice selection, click on any other option and that will change your answer.  A second choice is optional. If you want to 
                change your second choice selection, click on any other option and that will change your answer.  If you want to erase your 
                answer and have no second choice, then double click your answer and it will be erased (you may only need to single click this 
                seems to vary with browsers).
